Student Injuries at Clinical Agency

1. Seek testing, evaluation, and treatment as appropriate.
   
2. Fill out an incident report through appropriate agency department.
   
3. Provide copy of incident report to the Dean's office.
   
4. Contact Susan Taylor at 780-4133 if you have questions.

In emergency call 911. All accidents shall be reported to the USM Police. 
Also see Student Exposure Incident Report Form.

If a College Faculty or staff member is aware that a student accident/injury has occurred he/she should report the injury to the Dean's office as well (780-4133).

Deal with emergency 911 first, USM Police reporting, then submit a copy of report to the Dean's office.
