Process Recording and Assessment Paper

DUE:W Nov. 1 / M Nov. 13Class 9

Write a well organized coherent paper presenting an assessment and process recording based on your own practice in your field placement. The main body of the paper should be no more than 6 pages long. Provide a process recording of a critical incident in your work with an individual, family, or group as an appendix. The process recording may not be longer than 4 pages. Points will be taken off for excess length. Provide page numbering. Place your name on a blank page at the Back of your paper.

IProvide an introduction including the following:

A.Begin with a presentation of the context of the practice situation. This should include information concerning the agency function and general information about the population served. 

B.Follow this with a concise presentation of the unique situation of your client. Include information concerning your client’s route to your agency and to you (voluntary, nonvoluntary, involuntary). 

II.Provide the following concise assessments: 

A.Strengths Assessment (include environmental strengths and obstacles and personal strengths and obstacles (see Cowger in Saleebey). Include both abstract assessment of strengths and give concrete, specific example that illustrates the strengths.

B.Ecosystems Assessment using the outline provided by Miley, O’Melia, & DuBois on pages 236-237.

C.Brief Social History following the outline provided by Miley, O’Melia, & DuBois on pages 240-241.

III.Discussion

A.Compare and contrast your own point of view and your client’s point of view. Connect your statements to the position and power you hold in this situation. Connect your statements to the client’s unmet needs that are related to your interaction.

B.Discussion yourFormulation:Describe how your assessment directs what you say and do with your client(s).

1)Include your primary working hypothesis (educated guess) about what you think will be helpful for you to do with this client. Provide your reasons for choosing this hypothesis and the reasons you have chosen a particular approach to your work with this client. 

2)Provide a summary of your working contract - your agreement about how to work together. Remember to pay attention to issues of informed consent for social work intervention. 

IV.Provide a brief introduction to your process recording:

A.Describe the stage of your work together (e.g. 3rd individual meeting, contact within a milieu, 1st home visit).

B.Provide a clear statement of the purpose of the session.

C.Indicate your reasons for choosing a particular critical incident.

V. Critical incident process recording in the appendix. Use sample provided as your guide.
